Open Hearts AFH

Open Hearts AFH is licensed by Washington's DSHS as an Adult Family Home (AFH) for 6 residents and for Dementia and Mental Health specialties.

We have 3 private rooms and 2 semi-private.  Open Hearts AFH provides a safe and homey atmosphere for seniors with skilled caregivers and professional oversight. We provide an extensive activities program that takes into consideration the residents needs, interests and capabilities.

Open Hearts AFH and its staff are experienced with: Hospice and end-of-life  and managing residents who  are Wanderers, Exit Seekers or are Resistive to Care.

We have 24/7 awake night shift. We have a primary care physician available (House Doctor) who will make house calls and accepts Medicare.

We accepted private pay and Medicaid.

Open Hearts AFH is located in Bothell. Our residents typically come from the surrounding committees; Kenmore, Kirkland, Woodinville, Lake Forest Park, Lynnwood, Canyon Park, Brier, Mountlake Terrace, Redmond, and Shoreline . They come to us from; their own homes, retirement communities, assisted living, rehab facilities, nursing homes and other similar senior/elder care facilities.
